Free refusals register template
A printable refusals register for recording refused age-restricted sales, laid out with the columns used in the widely adopted No ID No Sale log format: entry number, date, time, product, name or description of the person refused, observations, the staff member who refused, and manager signature and date.

- Keep the register at the till so refusals are recorded immediately, not at the end of a shift.
- Fill in every column for every refusal — an incomplete entry is weak evidence.
- Record attempted proxy sales (an adult buying for someone underage) as well as direct refusals.
- Have a manager review and countersign the register regularly — weekly is a good standard.
- Keep completed pages at the premises; licensing officers and the police may ask to see them.
